Billing Frequently Asked Questions

Where do I mail my payment?

Ideal Rehabilitation Billing
7558 W. Thunderbird Road
PMB 408
Peoria, AZ 85381

How often will I receive a bill from your office?

We send out monthly statements however you will only receive a statement if your insurance has paid on claims.

Why is my statement different each time?

Our statements are generated each month from what payments have come in from your insurance company. If you have a balance it will just add to the previous balance.

Why did it take so long for me to receive a statement?

We have to wait to receive payment from your insurance company before we can bill you for your portion of the claim. Sometimes there is a delay from the insurance carrier because they have requested information from either the patient, another doctor or even our facility. Once they receive the information they will release payment to us and then we can bill you for your portion.

What if I cannot pay my bill in full?

Please just call our billing office so that we can set you up on a comfortable payment plan until your bill is paid in full. We DO NOT charge interest on any unpaid portion of your bill.

Why does my insurance company send me accident information forms?

With physical therapy and the Diagnosis codes they sometimes will check to make sure some other insurance is not responsible for the bills. Ex: Auto Accident, Workers Comp. Just be sure to fill them out and send back no matter what cause if they are not sent back in they will deny the claims and ask us to bill the patient.

Can you waive my Co-pays/Co-insurance or Deductible?

We cannot waive any co-pays/co-insurance or deductibles due to this is a violations of our contracts with the insurance companies and also with the Board of Insurance. But we can set you up on a payment plan if you cannot pay these in full.
